By signing, the PATIENT agrees to release IVY Wellness Center & Chiropractic Center, its owners, employees, and therapists from any liability, complaint, or claim arising from their therapy. This includes costs resulting from actions or omissions of the PROVIDER that cause damages or losses to the patient, therapists, and/or third parties. The PATIENT declares that they have been informed about the service/therapy to be provided at the IVY Wellness Center & Chiropractic Center by an independent and certified professional, hereinafter referred to as the PROVIDER, and acknowledges that they cannot hold the PROVIDER responsible for any adverse effects that may result from the therapy. The PATIENT also acknowledges that the IVY Wellness Center & Chiropractic Center has no employment relationship with the PROVIDER and simply rents a physical space for the PROVIDER to provide their services.
